Although in many cases they remain asymptomatic, over time they can cause severe pain and complications such as biliary colic, cholecystitis or jaundice. Fortunately, the risk of developing them can be significantly reduced through lifestyle changes and appropriate preventive measures.

What is a gallstone?
A gallstone is a crystalline formation that develops in the gallbladder or the bile ducts from normal or abnormal components of bile. Bile is a fluid produced by the liver that aids in the digestion of fats and is stored in the gallbladder. If the composition of the bile changes, the substances dissolved in it may precipitate and form stones.
Types of gallstones
Gallstones can be classified into several groups based on their composition. It is important to distinguish between them, as the underlying causes of their formation and the associated risk factors may vary.
- Cholesterol stones: This is the most common type, accounting for approximately 75% of cases. They form when bile becomes oversaturated with cholesterol. Normally, bile acids and phospholipids keep the cholesterol in solution. However, if the amount of cholesterol increases, or if the substances acting as solvents decrease, the cholesterol may crystallise and form stones.
- Pigment stones: These gallstones are dark in colour; their colour is due to bilirubin, a substance produced when red blood cells break down. There are two subtypes:
- Black pigment stones: They are generally associated with conditions in which there is an increased release of bilirubin. Examples include chronic haemolytic anaemia (e.g. sickle cell anaemia) or liver cirrhosis.
- Brown pigment stones: They most commonly develop in the bile ducts as a result of bacterial infections. Anatomical abnormalities may also predispose individuals to their development.
Causes and risk factors for gallstones
There may be a number of factors underlying the development of gallstones. Although the exact causes are not always known, research has identified several risk factors.
Risk factors beyond our control:
- Age: The risk of developing gallstones increases with age.
- No: Gallstones are two to three times more common in women, particularly those of childbearing age. This is due to the effect of female sex hormones (oestrogen), which increase the cholesterol level in bile.
- Genetic predisposition: There is a pattern of the condition running in families, which suggests a genetic cause (LITH genes). If a close relative has had gallstones, your risk is also higher.
- Ethnicity: It is more common amongst certain ethnic groups, such as Native Americans and people of Latin American descent.
Risk factors that can be influenced:
- Obesity: Being overweight and obesity are among the most significant risk factors. They increase the excretion of cholesterol in the bile, which promotes the formation of cholesterol stones.
- Rapid weight loss: Rapid weight loss following drastic diets or weight-loss surgery also increases the risk of stone formation, as the liver releases extra cholesterol.
- Nutrition: A Western-style diet, which is high in refined carbohydrates and fats and low in fibre, may contribute to the formation of gallstones.
- A sedentary lifestyle: Physical inactivity can slow down digestion and the emptying of the gallbladder, thereby increasing the risk.
- Certain medicines: Medicines containing oestrogen (e.g. contraceptives, hormone replacement therapy) and certain cholesterol-lowering drugs (fibrates) may increase the risk of developing gallstones.
- Pregnancy: Hormonal changes during pregnancy increase the risk of gallstones.
- Other diseases: Diabetes, insulin resistance, Crohn’s disease, cirrhosis of the liver and non-alcoholic fatty liver disease are also risk factors.
Symptoms and diagnosis of gallstones

Biliary colic: This is the most common symptom caused by a stone becoming temporarily lodged.
- Pain: The pain typically occurs in the upper right part of the abdomen or just below the breastbone (epigastrium). It often radiates into the back, between the shoulder blades or into the right shoulder. The pain gradually intensifies, reaches its peak, and then slowly subsides. A biliary colic attack usually lasts from 30 minutes to a few hours.
- Nausea, vomiting: The severe pain is often accompanied by nausea and vomiting.
- Sweating, restlessness: The patient is unable to find a comfortable position to relieve the pain and is constantly moving.
Acute cholecystitis: If a stone permanently blocks the duct leading out of the gallbladder, the bile that accumulates in the gallbladder may become secondarily infected, leading to inflammation.
- Persistent pain: Unlike biliary colic, the pain is constant and usually lasts for more than 4–6 hours.
- Fever: The inflammation causes a rise in body temperature and a fever.
- Touch sensitivity: The upper right side of the abdomen becomes tender to touch. Murphy’s sign is characteristic: when pressure is applied beneath the right costal margin whilst the patient is taking a deep breath, the patient suddenly stops breathing due to the pain.

When should you see a doctor?
Ha epegörcsre utaló tüneteket tapasztal, mindenképpen keresse fel háziorvosát vagy egy gasztroenterológus szakorvost. Azonnal forduljon orvoshoz, ha a fájdalom nagyon erős, több órán át tart, magas láz, hidegrázás vagy sárgaság kíséri. Lifestyle and diet for the prevention of gallstones
The key to preventing gallstones is a healthy lifestyle, centred on a balanced diet and regular exercise.
- Weight management Az ideális testsúly elérése és megtartása a legfontosabb lépés. Kerülje a drasztikus, gyors fogyást ígérő diétákat. A lassú, heti 0,5–1 kg-os súlyvesztés a biztonságos és javasolt.
- Kiegyensúlyozott étrend
- Eat foods rich in fibre: The high fibre content of whole grains, vegetables, fruit and pulses helps to lower cholesterol levels and supports digestive health.
- Choose healthy fats: Instead of saturated and trans fats (e.g. red meat, processed foods), opt for unsaturated fats, such as those found in olive oil, avocados, nuts and fish (omega-3 fatty acids). These can help ensure regular emptying of the gallbladder.
- Mérsékelje a cukor- és finomított szénhidrát-fogyasztást: A túlzott cukorbevitel növelheti az epekő kockázatát.
- Eat regularly: Ne hagyjon ki étkezéseket. A koplalás és a rendszertelen étkezés lassíthatja az epehólyag összehúzódását, ami az epe besűrűsödéséhez vezethet. Különösen fontos a reggeli.
- Regular physical activity A rendszeres, mérsékelt intenzitású fizikai aktivitás (pl. napi 30 perc séta, úszás, kerékpározás) segít a testsúlykontrollban, javítja az anyagcserét és csökkenti az epekő kialakulásának kockázatát.
- Megfelelő folyadékbevitel A bőséges folyadékfogyasztás, elsősorban víz formájában, segít az epe hígabb állagának fenntartásában. Napi 2-2,5 liter folyadék bevitele javasolt.
- Kávé és alkohol mértékkel Egyes kutatások szerint a mérsékelt kávéfogyasztás csökkentheti az epekő kockázatát. Az alkohol fogyasztása azonban növelheti azt, ezért csak mértékkel javasolt.
Treatment options
A tünetmentes epeköveket általában nem szükséges kezelni, elegendő az orvosi megfigyelés és az életmódbeli tanácsok betartása. Tünetek esetén a kezelés célja a fájdalomcsillapítás és a szövődmények megelőzése.
- Treatment of stomach cramps: A görcsös fájdalom enyhítésére elsősorban nem-szteroid gyulladáscsökkentők (NSAID-ok), például a diclofenac javasoltak. Ezek mellett görcsoldók is adhatók.
- Epehólyag-eltávolítás (cholecystectomia): A tüneteket okozó epekövesség végleges megoldása az epehólyag műtéti eltávolítása. Ma már a legtöbb esetben laparoszkópos eljárással végzik, ami kisebb sebekkel, gyorsabb felépüléssel és kevesebb szövődménnyel jár. Az epehólyag nem létfontosságú szerv, eltávolítása után a máj által termelt epe közvetlenül a vékonybélbe ürül, és az emésztés a legtöbb embernél problémamentesen folytatódik.
Következtetés: tegyen az egészségéért!
Az epekő gyakori, de nagyrészt megelőzhető probléma. Az egészséges testsúly fenntartása, a rostban gazdag, kiegyensúlyozott étrend és a rendszeres testmozgás kulcsfontosságú a kockázat csökkentésében.
